CagriSema is being studied in clinical trials and is not approved by the U.S. FDA. It is not legally available for human use outside an authorized clinical study.
Status as of July 23, 2026
The reported safety picture for CagriSema is dominated by gastrointestinal effects rather than by any new form of organ toxicity, with nausea, vomiting, diarrhea and constipation the events recorded most often across the phase 3 REDEFINE program. The combination is investigational, has no post-marketing record, and its label has not been written, so every statement below rests on 68-week trial data plus the established GLP-1 receptor agonist class framework rather than on years of population use.
Across the 68-week phase 3 REDEFINE program the reported safety burden of CagriSema was predominantly gastrointestinal, characterized as mild to moderate, and concentrated during dose escalation, with adverse-event discontinuation in the single-digit percentage range and no new form of organ toxicity identified.
Nearly every commonly reported event in the CagriSema trials came from the digestive tract, and the frequency gap between the combination arm and placebo in REDEFINE 1 was wide. The more informative contrast sits underneath that headline: serious adverse events, meaning events that were life threatening, required hospitalization or caused lasting impairment, were broadly comparable between arms, so the combination made far more people feel unwell without markedly increasing events that required emergency care.
In REDEFINE 1 gastrointestinal adverse events were reported by a clear majority of the combination arm against a much smaller fraction of the placebo arm, while serious adverse events were broadly comparable between the two.
The share of participants who felt unwell and the share who actually withdrew are two very different numbers in REDEFINE 1, and the distance between them is what tolerability data are really reporting. Discontinuation for adverse events sat in the single digits, higher than placebo, with gastrointestinal complaints behind most of it, while dose reduction absorbed much of the remaining pressure.
Adverse-event discontinuation in REDEFINE 1 stayed in the single-digit percentage range while a considerable share of participants finished the 68-week trial on less than the maximum dose, so dose reduction rather than withdrawal absorbed most of the tolerability pressure.
A naive additive model predicts that stacking two appetite-suppressing pathways roughly doubles the symptom burden, and the four-arm design of REDEFINE 1, which set the combination against semaglutide alone, cagrilintide alone and placebo, is what makes that testable rather than speculative. Reported gastrointestinal rates in the combination arm landed in a similar range to the semaglutide arm, pointing to a ceiling effect rather than a sum, since amylin signaling acts largely through the same area postrema and hindbrain satiation circuits and also slows gastric emptying.
| Criterion | Combination | Semaglutide alone |
|---|---|---|
| Gastrointestinal event rate | Similar range, not doubled | Reference arm |
| Injection site reactions | Reported more often | Reported less often |
| Satiation pathways engaged | Amylin plus GLP-1, overlapping | GLP-1 only |
| Evidence level | Human phase 3, 68 weeks | Human phase 3, 68 weeks |
In the four-arm REDEFINE 1 design gastrointestinal event rates in the combination arm fell in a similar range to semaglutide alone rather than roughly doubling, with injection site reactions the clearest distinctive contribution of the amylin component.
A resting heart rate rise on the order of a few beats per minute is an established property of GLP-1 receptor agonists, and the CagriSema trials reported a change consistent with that class pattern rather than anything unexpected. Whether a persistent increase of that size carries clinical weight is genuinely unsettled: elevated resting heart rate tracks worse outcomes in observational epidemiology, yet the large semaglutide outcome trials recorded the same effect alongside net cardiovascular benefit, which suggests a pharmacological signature rather than a marker of harm.
The REDEFINE trials were powered for weight and metabolic outcomes over 68 weeks, so any adjudicated cardiovascular events in them are descriptive rather than conclusive and formal proof of cardiovascular benefit for the combination does not yet exist.
Most of the formal warnings attached to this combination come from the semaglutide half rather than from its own trial data, because the regulatory framework built around GLP-1 receptor agonists already exists. Class warnings do not transfer automatically, though: a regulator writes the label for the specific product, so the final wording for CagriSema will be set through its own review rather than inherited wholesale.
GLP-1 class labelling contraindicates use in medullary thyroid carcinoma or multiple endocrine neoplasia type 2, and in January 2026 the FDA, after a meta-analysis of 91 placebo-controlled trials covering 107,910 patients, found no increased suicidality risk and asked manufacturers to remove that warning from class labelling.
Titration is the main lever this drug class offers on tolerability, and the REDEFINE trials leaned on it heavily. Both components climb in steps over several months toward the 2.4 mg plus 2.4 mg weekly target rather than starting there, because the receptors that mediate nausea adapt with repeated exposure and a gradual ramp gives that adaptation time to occur.
REDEFINE 1 allowed investigators to slow, hold or reduce the dose during the multi-month climb to the 2.4 mg plus 2.4 mg weekly target, which kept discontinuation in the single digits while leaving a large share of participants below the maximum dose.
Any substantial weight loss, whether from surgery, diet or medication, removes lean tissue along with fat, and incretin studies commonly place fat-free mass at roughly a quarter to a third of total weight lost, broadly in line with comparable non-pharmacological weight loss. The concern is therefore arithmetic rather than unique catabolism: the larger the total loss these drugs produce, the larger the absolute quantity of lean tissue that goes with it, and that matters most at the extremes of age and frailty.
Roughly a quarter to a third of total weight lost across incretin studies is fat-free mass, and the proposition that amylin signaling shifts that ratio favorably rests on preclinical work and small body-composition substudies that were not designed to prove it.
Neither component drives blood glucose down in a way that typically causes hypoglycemia on its own, since GLP-1 receptor agonists stimulate insulin release in a glucose-dependent manner and amylin analogues act mainly on gastric emptying, glucagon suppression and satiety. The risk enters through the company the drug keeps, which is where the documented episodes of low blood sugar in the REDEFINE 2 population originated.
The documented hypoglycemia in REDEFINE 2 occurred where the combination sat alongside insulin or a sulfonylurea, and the response described for this class is anticipatory dose reduction of those agents at initiation rather than adjustment after a first low reading.
The absolute exclusions follow the GLP-1 template and the list is short. Everything beyond it is a matter of clinical judgement rather than a flat no, and because CagriSema is investigational with its final label unwritten, none of it substitutes for a prescriber reviewing an individual medical history.
Under the GLP-1 template a personal or family history of medullary thyroid carcinoma, multiple endocrine neoplasia type 2, serious hypersensitivity to either component and pregnancy are absolute contraindications, and CagriSema remains investigational with its own label not yet written.
The evidence base is strong on the first year or so of treatment and thin on everything after that. A 68-week trial characterizes common side effects but cannot speak to cumulative exposure in a chronic condition treated over years, and a few thousand participants cannot surface an event that occurs in one person in ten thousand.
CagriSema has been submitted for regulatory review rather than established in routine practice, so its final labelling, its safety beyond the 68-week trial horizon and its rare-event profile all remain undetermined.
